Request a speaker for your group or organization

Trellis Supportive Care and Wake Forest Baptist Health have come together to promote a common cause in the Piedmont Triad region of North Carolina. The mission of the collaborative has been to “encourage conversations about end of life care.” Working closely with other healthcare partners, one of the primary goals became the creation of a simplified advance directive for North Carolina and a regional initiative for advance care planning.

 

Advance care planning is a process where you reflect upon and plan for the healthcare you would want if you are unable to make decisions or speak for yourself.  Got Plans? Workshops:

 

  • Dialog about importance of this process: Decide, Discuss, Document.

  • Review important documents:  Living Will, Healthcare Power of Attorney, Medical Order Scope of Treatment (MOST)  forms

  • Offered monthly- see the Got Plans calendar ( www.gotplans123.org )

  • Offered to groups of 15/or more attendees.

  • Groups requesting the “Got Plans” workshop are asked to provide a notary so that attendees can complete their documents that day.
