What is advance care planning?

 

  • process where you reflect upon and plan for the care you would want if you are unable to make decisions for yourself.

  • Share this information with loved ones and put your wishes in writing.

Why is this important?

 

  • Advance care planning helps your loved ones and healthcare providers honor your goals and wishes if you are unable to speak for yourself.  

​

  • If you are ever in an accident, have surgery, or get a serious illness, you may need someone to advocate for your wishes.
