What is advance care planning?


  • process where you reflect upon and plan for the care you would want if you are unable to make decisions for yourself.

  • Share this information with loved ones and put your wishes in writing.

Why is this important?


  • Advance care planning helps your loved ones and healthcare providers honor your goals and wishes if you are unable to speak for yourself.  

  • If you are ever in an accident, have surgery, or get a serious illness, you may need someone to advocate for your wishes.

Our Community Outreach programs, including GotPlans™ advance care planning workshops, are being handled virtually through Zoom due to the social distancing recommendations during the COVID 19 outbreak. We are pleased to offer this workshop to the community, free of charge, each Tuesday from 1-2pm.
